Complete the Interest Form

Complete the Surgical Corps interest form to be added to our database.

Sign the Docs

1.) BCM Enrollment Certification (link to form)

  • Certifies that you are a full-time Baylor College of Medicine student
  • Print out the form, fill in only your name in the blank, take it to the registrar’s office and have an associate sign it.

2.) Surgical Corps Student Agreement (link to form)

  • The official agreement between Surgical Corps and its independent contractors.
  • Print out form and fill in the date and your name on page 1, your address on page 4, and sign/print/date the section under “Independent Contractor” on page 5.

3.) Methodist Badge Request Form (link to form)

  • Download the document and fill out your “Name”, “Telephone Number”, and “SSN ” (social security) on the top right.
  • E-mail it to Angela Mitchell at ALMitchell@tmhs.org. She will e-mail it back with the authorized signature.
  • Deliver a copy of the signed form to the Human Resources department in Methodist Scurlock Tower (Basement level 1) from 7:30am – 4:30pm, Monday – Friday.
Deliver the BCM Enrollment Certification and Surgical Corps Student Agreement you completed in steps 1 and 2, respectively, to Rishi Kumar’s mailbox (#100) on the 2nd story of the DeBakey building. You will receive a digital copy of the forms for your records.
